It doesn't get default when inserted. I should have removed this option, because it's outdated, but keep it because many used it early on. I recommend to use the "Set featured image" functionality instead. It's much better since it can be exported/imported and images can be added from the media library. External images can be added to the "Image URL" field below the editor. You don't have to insert the image when using the "Use First Attached Image as default" setting either.
